Summit Counseling, PLLC is not paneled by insurance. Appropriate forms can be issued if you wish to submit to your insurance carrier for reimbursement.
Accessing Your Health Records
Under Texas law (Section 181.105 of the Health and Safety Code), you may request access to your health care records maintained by this practice. Requests are processed in accordance with applicable state and federal law, including timelines and permitted fees. To request your records: Submit a written request via email found on our Contact page. Please include your full name, date of birth, and a description of the records you are requesting. Specify your preferred method of delivery (PDF via email, mailed copies, etc.). We will respond and provide access as required by law.
